Athlete Summer Registration 2020

Please complete the following 2 steps for MOBALE Athlete Summer Registration by June 1, 2020:

1. Complete the forms in the  Athlete Summer Application Package 2020

2. Complete the 1 to 3 Day USABA Sports Club Event Membership

PLEASE NOTE!! When completing the above Event Membership For the date(s) of the event, type "6/17/20"; then in the comments box, write “for event 6/17 at 1 PM to 6/21 at 2 PM."  

 3. Complete the Disabled Sports USA Release form

4. Complete Maine Adaptive Sports On The Road Events Demographic Survey -- this is an important part of their grant funding, and all the information stays exclusively with them.
